It includes sections for confirming participation, providing basic contact information, and explicitly addressing data privacy and GDPR-related concerns. Crucially, it contains a section for collecting "additional personal information" (such as year of birth, education level, employment status and language skills) to ensure the assembly represents a diverse cross-section of society.
